Virtual/Hybrid Lunch Order Forms

VIRTUAL SCHOOL LUNCH OPTION
 
Each Monday, virtual school parents may come to their child's home school to pick up meals. Any parent who wishes to have 10 meals provided (5 breakfasts and 5 lunches) for their student, should complete this form no later than Friday before the upcoming week. The district will charge student accounts for the 10 meals at the time of pick-up for any reimbursable meal for students. Students who qualify for free breakfast and lunch through the USDA National School Lunch Program will not be charged for any meal. Students who qualify for reduced meals through the USDA program will be charged at the reduced price meal rate. If your child is enrolled in a Community Eligible Provision School (CEP), there will be no charge for meals but you still must complete a school lunch request to receive breakfast and lunch.
 
 
 
 
 
HYBRID STUDENTS LUNCH OPTION 
 
Parents may select for their child to receive a meal to take home while the school district is operating on a hybrid schedule. Students who attend school on Monday and Wednesday can receive breakfast and lunch for Tuesday, Thursday, and Friday. Students who attend school on Tuesday and Thursday can receive breakfast and lunch for Monday, Wednesday, and Friday.

The district will charge student accounts for the meals at the time of pick-up for any reimbursable meal. Students who qualify for free breakfast and lunch through the USDA National School Lunch Program will not be charged. Students that qualify for reduced meals through the USDA program will be charged at the reduced price meal rate.
If your child is enrolled in a Community Eligible Provision School (CEP), all children will receive a meal on remote learning days and this form does not need to be completed.
